NIST AI Risk Management: Your ISO 14971 Already Has the Answer
NIST published over 400 pages on AI risk management. Your ISO 14971 risk file is 30 pages. Guess which one your FDA reviewer actually reads.
The AI Risk Management Framework (NIST AI 600-1) landed with a thud in regulatory affairs departments across MedTech. Another framework. Another acronym. Another mountain of guidance to absorb, interpret, and somehow wedge into an already overloaded quality system.
But here's what most of the panic misses: if you're already doing ISO 14971 well, you have 80% of the AI risk management architecture you need. The gap isn't framework. It's application.
The NIST AI Framework in 5 Minutes
NIST AI RMF organizes AI risk management around four core functions:
- GOVERN — Establish policies, roles, and culture for responsible AI
- MAP — Contextualize the AI system and identify risks
- MEASURE — Assess and track identified risks
- MANAGE — Prioritize and act on risks
If you're an ISO 14971 practitioner, this should sound familiar. Because it is.
The Mapping You Didn't Know You Already Had
Here's where it gets interesting. The NIST functions map almost directly to your existing 14971 process:

Four out of five NIST functions have direct 14971 analogs. The fifth — GOVERN — maps to your management review process (ISO 13485 Section 5.6) and your quality policy framework.
You're not starting from zero. You're extending what you already have.
The "New" Concepts That Aren't New
The AI risk conversation introduces terms that sound alien to traditional MedTech quality teams. They're not. They're extensions of concepts you already manage:
Algorithmic Bias → Foreseeable Misuse (14971 Clause 4.2)
Bias is a hazard. A diagnostic AI that performs differently across demographic groups is producing foreseeable harm. You already have a process for identifying foreseeable hazards — now add demographic performance variance to your hazard identification checklist.
Transparency/Explainability → Residual Risk Communication (14971 Clause 8)
When a clinician can't understand why an AI reached a conclusion, that's residual risk. Your IFU already communicates residual risks to users. For AI-enabled devices, add: what the model does, what it doesn't do, and when the clinician should override it.
Model Drift → Post-Production Information (14971 Clause 9)
A model that degrades over time is generating new hazardous situations in the field. Your post-market surveillance process already monitors for this. Add: model performance metrics, retraining triggers, and drift detection thresholds.
Accountability → Management Responsibility (ISO 13485 Section 5.1)
Who is responsible when the AI is wrong? Your management review process already assigns responsibility for quality system performance. Extend it: designate AI risk ownership, review AI performance data at defined intervals.
What 14971 Genuinely Doesn't Cover
I'm not going to pretend 14971 handles everything. There are legitimate gaps:
1. Sociotechnical Risk
Traditional 14971 focuses on patient safety. NIST AI RMF includes broader societal impacts — equity, access, environmental cost. Your risk file needs a new section.
2. Continuous Learning Systems
A locked algorithm is easy to validate. An algorithm that learns from new data is a moving target. Your 14971 process assumes the device stays the same after market release. SaMD with continuous learning breaks that assumption. You need: revalidation triggers, performance monitoring, and predetermined change control plans.
3. Data Governance
Training data quality is not a 14971 concept. But garbage in, garbage out is a real hazard for AI systems. Add: data provenance, representativeness assessment, and ongoing data quality monitoring to your risk management file.
4. Human-AI Interaction
How does the clinician interact with the AI output? Does the AI augment human judgment or attempt to replace it? This interaction design is a risk factor that traditional hazard analysis doesn't always capture.
These are real additions to your risk file. But they're additions, not replacements. The architecture holds.
What to Do Monday Morning
If you're a risk management lead at a MedTech company evaluating AI/ML-enabled devices:
- Pull your current 14971 risk management plan for any SaMD or AI-enabled device
- Map it against the table above — identify which NIST functions you already cover
- Add the four gaps (sociotechnical risk, continuous learning, data governance, human-AI interaction) as new sections in your risk file
- Update your hazard identification checklist to include AI-specific hazards: bias, drift, opacity, data quality
- Bring the updated risk file to your next management review and discuss AI risk ownership
That's a week of work, not a year. And it positions your quality system for the convergence that's coming: NIST, FDA's AI/ML action plan, EU AI Act, and IMDRF guidance are all pointing the same direction.
The Bottom Line
Stop treating AI risk management as a separate discipline. It's a 14971 extension.
The companies that figure this out first will spend less time building parallel risk frameworks and more time actually managing risk. Which is, after all, the point.
